Geriatric Services

Geriatric Services for the older adult is a special focus at CHC both in the office and in the community setting. Offered are:

●Office visits scheduled with sufficient time to attend to both simple and complex health care issues.

●Focused consultations regarding Alzheimer’s care and care giving needs.

●Inpatient assisted living and long term care at our associated facilities.

●Other consultation services as requested.

Medicare accepted for age 65 and older, for applicable services.
